Engine cold , unscrew the O2 from the exhaust but leave it connected , turn ignition ON and (carefully) try with you're fingers - is it heating up or not ..

That's a really good shout, while it has continuity it might not be heating up. Will give it a go shortly

One quick thought while removing the lambda sensor. Mine is way down past the gear linkages, could this be too far away for a good reading? I'm wondering would it be better to move it closer to where the 4 branch goes from 4-1.

Shouldn't be an issue with heated O2 , but personally i'd rather relocate it closer to the manifold ie. just near the end/exit of the exhaust manifold collector .
